Holland Behavioral Health LLC (“Holland Behavioral Health,” “we,” “us,” or “our”) respects your privacy. This Website Privacy Policy explains how we collect, use, and share information when you visit hollandbehavioralhealth.com and any pages, forms, or appointment-request tools we operate on that domain (the “Website”).  This policy applies only to the Website. It does not apply to third-party sites we may link to. It also does not replace the HIPAA Notice of Privacy Practices that governs protected health information once you are a patient of the clinic.


Do not send detailed health information through an ordinary website form or unsecured email.


Please read this policy before using the Website or sending us information through it. We may update it from time to

time. The effective date above will change when we do. Continued use of the Website after a revision is posted means

you accept the updated policy. If you do not agree, do not use the Website.

2. This policy vs. HIPAA

Information you type into a public website (name, email, phone, a short appointment request) is treated under this Website Privacy Policy.  Protected health information created or received in the course of treatment — assessments, diagnoses, medication records, clinical forms completed as a patient — is governed by HIPAA and by our Notice of Privacy Practices, which we provide when you become a patient. Those records are not used for website marketing.


Please do not include diagnoses, medication lists, or other clinical detail in the Website contact form. Use the new-patient process we send after an appointment is scheduled, or call the office.


3. Information we collect

Information you give us:

We collect information you choose to submit, including when you request an appointment, complete a contact form, email or text the clinic, or call us from a number listed on the Website. That may include your name, email address, phone number, preferred contact method, preferred days or times, and any message you write.


Information collected automatically:

Like most sites, our hosting provider, analytics tools, and security services may automatically record technical data when you visit: IP address, browser and device type, operating system, referring URL, pages viewed, dates and times of visits, and general location derived from IP (city or region, not precise GPS unless you separately allow it). We use this to keep the site working, understand which pages are useful, and detect abuse.


Cookies and similar tools

The Website may use session cookies (deleted when you close the browser) and persistent cookies or similar technologies to remember basic preferences, measure traffic, and protect the site. You can set your browser to refuse cookies or to alert you before they are stored. Some features may not work if cookies are blocked.


Location

We do not need precise device location to run this Website. If a future map or booking feature asks for location permission, you can decline in your device settings. Approximate location from IP may still appear in server logs.

We may need to verify that the request comes from you. We may keep information when the law requires it, when it is part of a designated record set under HIPAA, or when we need it to complete a transaction you started.  To refuse cookies, use your browser settings. To stop optional email from the practice, reply or email the address above.  If you do not want us to collect information through the Website, do not submit forms and limit your use of the site.


You may always call the office instead.


7. How we protect information

We use reasonable administrative, technical, and physical safeguards appropriate to a small clinical practice and a public website. That typically includes encrypted connections (HTTPS/TLS) for the Website, access limited to people who need it, and hosting behind standard network protections.  No website transmission is guaranteed secure. Do not put detailed health information in a public form. If we later give you a patient portal or password-protected feature, do not share your password, tell us promptly if you think it was misused, and sign out when you use a shared computer.


8. Links to other sites

The Website may link to maps, crisis resources (including 988), insurers, or other third parties. We do not control those sites. Their privacy practices are their own. Leaving our Website is at your own risk.


9. Children

The Website is intended for adults who are seeking care or information about the practice, including parents or guardians arranging care. We do not knowingly collect personal information through the Website from children under 13. If you believe a child submitted information here, contact us and we will delete it from Website systems.


10. Retention

Website inquiry records are kept only as long as needed to respond, run the practice, meet legal or insurance requirements, and resolve disputes. Clinical records, if you become a patient, are retained under Kentucky law and our medical-records policy — not under this Website policy.

We may revise this policy. The new effective date will appear at the top. Material changes will be posted on this page.  Your continued use of the Website after posting means you accept the revised policy.
